At Esteem Carehome, we help people to live fulfilling lives, build confidence and develop the life skills they need to realise their potential and establish the foundations for independent living and to integrate with the community. We provide a safe and secure autism friendly living environment, working closely with residents to develop and deliver a person centred care and support plan to help make the changes needed for a better life.

 

Accommodation

Esteem Carehome is located in Greenford, a new location offering a range of accommodation for people with various support needs, and is within walking distance of shops, bus stops and leisure facilities. Spread over two floors, there are 3 spacious rooms. The house has a spacious kitchen, dining room and a beautifully decorated private lounge, TV room. Residents can socialice in two beautiful lounges, TV room and gardens.

Support

We have extensive experience in supporting people with learning disabilities and mental health needs to develop their skills and live as independently as possible.

Our structured package of tailored support, agrees a personalised support plan helping residents to identify their needs and aspirations.  Their progress is regularly monitored and reviewed, with the support plan updated to reflect any changing needs. Residents are encouraged to take control of their lives by building on their skills and strengths.

Our highly-trained staff offers a wide range of tailored support, advice and assistance, including:

  • Maintaining tenancy
  • Dealing with correspondence
  • Day to day general needs
  • Health and safety in the home
  • Cooking, cleaning and shopping
  • Socialising and community involvement
  • Managing finances
  • Accessing education, training
  • Support with employment
  • Regular Activities: cinema, swimming, gym, bingo

 

Social Space

Esteem Carehome has been designed based on the needs of adults with ASC including the need for large, low-arousal spaces and  opportunities for integration alongside time and space away from others. It provides the variety and volume of different spaces needed to reduce anxiety levels of individuals being supported and to allow efficient de-escalation and redirection of potential and actual behaviour which may be described as challenging.

It enables individuals to develop significantly greater independent and daily living skills including cooking, cleaning and domestic skills within a safe and homely environment. Residents can socialise in the sensory room and gardens. 

 

Referrals and Contacts

We provide a rapid response to referrals and will normally arrange a professional assessment within a few days of your call.

A full and detailed pre-admission assessment is always carried out, free of charge and without obligation, by our highly experienced team prior to a placement offering being made. This values the unique and complex needs of each person and forms the basis of the individuals support plans if they move into the service.

 

How to Apply and Eligibility

We accept referrals from the Ealing, Hounslow, Hillingdon, Harrow and Brent Council Social Care Team.

All care and support needs will be assessed before an application is accepted.

Applicants must be aged 18-65 and have a diagnosed learning disability or mental health need.
