Description

This service supports adults with severe and enduring mental health conditions where current accommodation is not meeting needs, or where the individual is homeless or threatened with imminent homelessness. In line with the Putting People First Agenda, the Service provides a space, free of discrimination where individuals are supported to improve their physical and mental health, well-being and quality of life. Support staff are on hand 24 hours a day and work with individuals to ensure they have control to make decisions in relation to their own lives; receiving a personalised service shaped by the individual themselves to ensure dignity, courtesy and respect.

 

This service operates from two buildings: Heather Lodge which is the high support element of the service and Glaucus Street which is the step-down element of the service. The service provides a steppingstone from various forms of institutional care e.g. residential care, from or to other supported accommodation and/or discharge from hospital to support individuals towards a more independent living.  

 

Heather Lodge is a 21-bed supported accommodation scheme that consists of 1 bed self-contained flats which comprise living room, kitchen, bathroom and bedroom. This section of the service focuses on engaging with and assisting individuals to make good use of community mental health services available to them whilst supporting them towards independent living.

 

Glaucus Street is an eight-bed block with 8 units of 1 bed self-contained accommodation which includes living room, kitchen, bathroom and bedroom. The service is situated within the same grounds as Heather Lodge (across the courtyard) and provides outreach support to individuals who move from higher support services to their own accommodation within the community. The focus of the service is to support individuals to live as independently as possible while enabling full engagement in their local community.
