Craegmoor is the leading independent support provider for individuals with enablement needs. We services to adults and young people over 16 years. Specialists in Autism specific Services, Complex Needs and Positive Behavioural Approaches. The main service user needs we support are mental health, learning disabilities and autism.
Supporting You in Rotherham
Supporting You in Rotherham aims to support adults and young people with learning disabilities to live the lives they want, in their own homes, in the community. We can support individuals in their own homes to develop independence and reach their full potential. Our support packages are identified around the support an individual needs, whilst our focus is on enabling people to achieve their goals and to become more independent providing the opportunities they deserve. The service offers specialist support to empower and encourage individuals to develop their self esteem and the skills they need, working towards a level of independence which is consummate with their skills, confidence and aspirations. Amongst the services available at Supporting You in Rotherham are:
• Specialist support for people with learning disabilities, autism, mental health illnesses, complex needs
• behaviours that challenge, to live the life they want
• Domiciliary care and support with aspects of everyday life, personal care, healthy eating, medication, maintaining the home, paying bills, shopping, budgeting and maintaining tenancies
• Support to access benefits including Direct Payments and Personal Budgets
• Community outreach services, support with becoming more confident, participating in community related activities and maintaining family contacts
• Improvement in quality of life irrespective of illness or disability
• 24 hour support available if needed providing choice, participation and inclusion
• Support to access education, voluntary and paid employment opportunities
By encouraging people to take control and have belief in their potential, we can make positive outcomes a reality for everyone, regardless of age or presumed ability, by creating pathways that support individuals through residential to community living, delivering measurable results for both local authorities and the people who use our services.
At the heart of our services, individual care pathways fundamentally shape our offerings so that we provide person centred approaches that encompass each individual’s needs to achieve positive outcomes and a transition to more independent living. This, paired with personalisation agenda, enables us to help the people we support to lead fulfilled, independent lives. And by working together, we can make a real difference.
Finding the right care and support may seem complicated, but at Craegmoor we can offer advice about accessing benefits and funding and if appropriate, working out your budget. If you have an Individual Budget or Direct Payment we can support you to decide how best to spend your finances to make sure you can live the life you want. Referrals may be made through:
• The individual’s Social Services Department management team or, if relevant, their local health authority
• By the individual (or an appropriate adult acting on their behalf) who wishes to use Direct Payment or Individual Budget, or self-financing
Cost
The cost of £13.50 is an hourly base fee.
