Optima Care's Shine model of therapeutic care offers 2 - 5 year person-centred pathways, designed to help young people transition into adulthood - and supported living with a sense of confidence and optimism.

Underpinned by the individual's Education, Health and Care (EHC) plan, Shine is tailored according to the individual's needs and helps ensure a transition journey through our services towards more independent living.

Moving on focused
- A service designed, built and managed by senior individuals with a wealth of experience in children's and adults services.
- Continuation of Education, Health and Care (EHC) plans.
- Continuity in communication systems and tools already practised by an individual. We promote a total communication environment, have strong links with schools, speech and language therapists and advocacy services.
- Life skills training and development.
- Person-centred transition programmes to help individuals move on to supported living post-25, either with Optima Care in partnership with a Registered Social Landlord or another provider.
