Description

We provide? 

The Shared Lives scheme facilitates long term placements, short breaks or day sessions in a Shared Lives carer’s home and community.

Who is it for?

Age – 16 years and upwards who meet the national eligibility criteria.  (applies to all service users).     

Adults with learning disabilities – who are not able to live independently e.g. who need care and /or support most of the time. 

The scheme provides long term placements and short breaks in Shared Lives carer’s home

Physical disability – short breaks for people who have a physical disability in Shared lives carer’s home.

Older people – The scheme is able to accept referrals for day time support (sessions) or short breaks in the community in Shared Lives carer’s home

Adults with mental health needs who require support some of the time. The scheme can provide long term placements or short breaks in Shared Lives carer’s home.

Adults with continuing health problems, e.g. long term conditions. The scheme is able to accept referrals for day time support (sessions) or short breaks in the community in Shared Lives carer’s home.   

When does it take place?

A long term placement can be 24 hours a day, 7 days a week. Short breaks and day sessions as assessed and agreed with the individual person. 

How much does it cost?

We provide a service that is identified through a person-led assessment which meets the national eligibility criteria. For more information on a person-led assessment, which includes a financial assessment, please call Kirklees Council Gateway to care on 01484 414933.
