Transitions
Sometimes people need support with change and the many transitions that we each go through in our lives. Whether it’s a transition back home from a hospital, from school into adulthood/services, to a new home, back into work or starting something new and unfamiliar. We can offer that one to one support for a smooth transition. Supporting you to voice your wishes, dreams and aspirations, we will be the voice for you. We are here for you and will be with you every step of the way, until you tell us that you no longer need our support.
A brief description of support with a transition from school to adult services:
· Complete a Needs Assessment when its identified you need support with your transition to adult services.
· If parents request respite or support with other services, the assessment will be conducted in a multi-disciplinary manner, so all the relevant services can input and prepare together.
· You and your parents can visit the services you are looking to use in the future.
· We will request that our assessment worker is invited to any children’s service review.
· If parents wish to pursue our service, a pre-admission assessment will be completed directly with you. The purpose of this assessment will be to gather specific details about your support needs, for example managing risks, behaviour, strategies, level of staffing required for specific tasks, i.e. bathing/showering, equipment used, length of time tasks take, medication needs etc.
· From the pre-admission assessment we will inform you and your parents/Social Worker if we can meet your needs and the support that will be required to do so.
· Any referrals required following the outcome of the pre-admission assessment would be made, for e.g. SALT, OT, behavioural support etc.
· Any telecare required to support you would be identified and sourced.
· Any additional training our staff require to support you will be identified and sourced.
· Our transitions worker will regularly meet with you and your family.
Our transitions worker with the key worker from your current service will work to establish and maintain frequent contact together with commencing the transfer of information.
