At the light we understand that returning home to continuing life with a long-term condition can be an extremely challenging time. We have an experienced healthcare team who are trained to the highest calibre.

We make sure we listen to your every need to make the transition back to home life as stress-free as possible.
All our health care professionals have up to date mandatory care training and would undergo any specialist training required for them to be involved with your tailor made package of care.

Our continued dedication to provide for your individual needs means we also employ our own specialist nurses as part of your staff team, who will come out to regularly asses the package of care and provide specialist advice and support.

Epilepsy management
Diabetic management
Administering medication
Gastronomy and non gastric feeding
Catheter care and Bowel management
Home ventilation and respiratory Management
Brain Injury and Spinal Injury rehabilitation

Following a thorough assessment of your needs we create an in depth person centred care plan ensuring the focus is on you, your medical needs and providing the best care to maintain your health & well-being.
Your care plan will then be reviewed regularly to ensure that we continuously adapt our service to meet your changing needs and personal preferences.

We provide this service for people who are being discharged from hospital, those who have ongoing nursing and clinical care needs that need to be continually monitored. People who have a degenerative health condition.

Those that find themselves faced with a long-term recovery process from an illness or accident, or people with a physical disability.

***Please note: All of our services have a 2hr per visit per week minimum commitment*** and the price shown is a typical cost for this service and is dependent on your individual needs and requirements. Once these have been discussed we can confirm a price.
