Description

We will make sure you have access to the health services you need and exercise our duty of care to safeguard your good health. We can work with you, your family, carers, medical services and others to ensure your healthy living and agreed courses of medical treatment.

If you have been prescribed medication we will ensure you receive the right dose at the right time and we will help you to understand the purpose and likely effects of your medication. Where appropriate, we will support and enable you to be able to administer your own medicines. We will meet with you to agree a support plan that meets your needs and gives you choice and control and we will review this regularly with you to make sure that as your circumstances change we can change with you.

Price
