Description
NCCN was established 20 years ago, it is a nurse led organisation with specialists in palliative care, learning disabilities, cardiac care, head and spinal injuries and behavioural challenges. We provide specific training so that staff can meet the needs of people who use the service, often working with health colleagues for complex services.
We provide domiciliary services across East Yorkshire, North Lincolnshire, Selby area and the City of Hull.
LEARNING DISABILITY SERVICES
We work with Local Authorities in the North East of England and specialise in large packages of care which are funded by Health Authorities, Insurance Claims and through Individual Budgets. The team is lead by Lyn Hole who is a Registered Nurse for people with Learning Disabilities, Lyn has specific interest in the Autistic Spectrum and has lots of success in working with people who have behavioural challenges.
Our service is flexible, to meet your needs. You will have your say and take part in the recruitment process if you would like to. This helps you to stay in control but takes away the hassle of doing payroll and other personnel tasks. We have insurance to cover your care package (up to £10M). Staff training is specifically tailored to your needs to make sure that you are safe and everyone knows how you like your care to be delivered. You will work with our assessment team to write your support plan which can be changed at any time.
We can work with you where ever you live and give you whatever level of support you need. You may need support such as:
• Someone to go to social activities with you
• Help around your home with general day to day tasks
• Assistance with making your needs known
• To make sure that you are safe when you are out and about
• To be with you when you go to the Day Centre
We are also able to provide Independent Supported Living where you would have your own tenancy and live independently in your own home with as much support as you need, even if you need 2 support workers to help you to live your life as you choose.
If you live in your own home, or would like advice and support to move into your own accommodation, we can work with you and your family to help you make this happen. We will then support in your own home so that you are able to make friends, go to work, attend any groups you would like to and go on holiday either on your own or with your friends.
We are used to working with people who choose to use their Individual Budgets, or Individual Support Fund in different ways and we will help to find new activities that you enjoy. Our Accountant will control the financial aspects and send you a statement every month which will let you know how many hours you have used and what you have left to use. You will be able to save up hours so that if you would like to have support whilst you have a holiday it is very easy to pay for it because the money is still in the pot.
