Description
Palliative Care focuses on the management of symptoms and problems associated with life threatening and life limiting serious illness. We understand that this can be a painful time for all concerned.
Our palliative care service aims to support during this difficult time and make life more comfortable for the individual, family and carers. Our goal is to increase comfort, promote dignity and provide a support system to the person and those close to them.
Chestnut Homecare provide a Palliative Care for anyone over the age of 18 with a serious illness that may be approaching end of life. We employ staff with the necessary skills in providing support and assistance to a person with palliative care needs.
We take a sensitive approach and develop an agreed care/support plan that takes into account a persons preferences and choices. We ensure that an individual is always treated with dignity and respect.
We fully understand how difficult it can be to have staff visiting a persons home (sometimes several times a day) and how intrusive this can feel. Additionally, we feel strongly about continuity of care and therefore, we only select a small amount of care staff to get to know a person and their lifestyle choices. In supporting a persons needs, Chestnut Homecare work closely with District Nurses, Mc Millan nurse specialists and many other health and social professionals.
We are able to offer palliative care during the day or night. This may include personal care, carrying out non-nursing duties, social, emotional and spiritual support.
We offer a no-obligation, free of charge initial meeting to discuss requirements. Our hourly rate is £13.00 for palliative care and there are no hidden extras.
