Woburn Sands Lodge has been established to provide a service which empowers individuals to feel sufficiently skilled and confident to embark on their next stage of recovery and a journey towards living in a smaller residential care placement and then towards increased independence.

Woburn Sands Lodge is registered to provide nursing support to adults with mental health and complex needs. Providing twenty four hour nursing support. This Nursing Support service have been established to provide a service which empowers individuals to participate in a clinical and social model allowing the people we support to plan for their future and develop their aims and aspirations.

We provide:

• A continuum of care: providing a nurse led assessment and therapeutic service for hospital or secure unit discharge, allowing for a rehabilitative route towards residential care onto supporting living.
• A test-ground for hospital discharge: providing the security of assessment, pharmacological monitoring, and support for acute symptoms in a nursing skilled environment. Woburn Sands Lodge offers hope to those who may be subject to the ‘revolving door syndrome’.
• Relapse intervention: providing nursing support where Mental Health relapse threatens residential care placement. Offering a less traumatic and less self-defeating option than re-admission to hospital.
• Flexible Support: providing long term placement for those with complex Mental Health needs or providing a planned and supported step down to a community placement.
•  Therapeutic nurse-led intervention: providing support for established clinical programs in liaison with multi-disciplinary teams/clinicians, e.g. anxiety management, CBT and psychological work to address medication noncompliance.
• Risk Assessment and Management focus: providing a nurse-led ‘Supportive Observation’ framework for risk management.

Staff

Woburn Sands Lodge have a history of individuals stepping down into more independent living such as supported living, as a result of their own progress with determination, supported by an experienced staff team.Our staff team pride themselves in putting the individual at the centre of their support and are renowned for their positive, empowering and caring approach. All staff undergo continual training and development, and specialist training is provided for bespoke individual support needs as part of the assessment and transition process.

Accommodation

A large house, Woburn Sands Lodge can accommodate 10 adults. Woburn Sands Lodge features several ground floor rooms, which maybe suitable for individuals with a mobility impairment, depending on individual needs. Two communal areas give each person the chance to interact with others, with one lounge featuring a small kitchenette suitable for those wanting to make light snacks. Elsewhere a large and modern kitchen is the central hub of the home where people are encouraged to participate in meal times with many helping to prepare and cook food for others.

Outside Woburn Sands Lodge is a well maintained garden where individuals look after the flowers and plants, as well as sunbathe and socialise during the summer months. A BBQ area also provides evenings of good food and enjoyment for all who wish to participate.

Location

A bus ride away from the town centre of Milton Keynes, Woburn Sands Lodge is in an ideal location for people who wish to increase their independence and social interaction. The town centre contains many shops and activities, but for those who prefer to stay local, a host of charity shops, restaurants, cafes and a college.

Clinical

We have nurse lead clinical and behavioural specialists to advise on emotional support and complex health needs. All therapeutic input is integrated into daily functional measured goals in line with the individual’s goals and aspirations. Progress is monitored through recognised outcome measures and reviewed regularly by the multi-disciplinary team.
