Home based physiotherapy for people with neurological conditions. Assessment and treatment at home and exercise packages set up for you to carry out. Gentle treatment to maintain and improve your function and mobility in your home environment.

Assessments will be carried out at home and will usually last an hour. This will involve discussion with the patient regarding their personal details, medical history and what they are hoping to achieve from physiotherapy.  Following this, a physical assessment of the patient will be carried out concentrating on how the problems identified in discussion present in daily life.  A set of goals to be achieved will then be set between the physiotherapist and the patient, so that both parties understand further ongoing treatment. Further follow-up sessions will work on these goals and they will be reviewed with both parties at specified times. Assistance can also be given on the provision of some aids to enable patients to remain safe at home.

This service is available Monday - Friday 9am-3pm.
