Description
Accommodation and support for individuals with learning disabilities and complex needs, including mental health issues, who have a history of offending and are ready to 'step down' from secure or formal settings as part of their recovery. We also support people who have had multiple unsuccessful placements within residential, supported living or in-patients services.
Optima Care's Shine model of therapeutic care provides the framework for our services and is applied by our expert support team, including our in-house MDT, with a view to delivering one overriding and ultimate goal: person-centred supported living for individuals in their local communities, close to loved ones.
Recovery Pathways
- Service and support is built around the needs of each and every one of our individuals, from functional assessments and total communications to milestone setting and outcome measures.
- Person-centred pathway content is led by our individuals and supported by family, friends, staff and carers, with ongoing input from our in-house MDT.
- The full MDT includes a clinical psychologist, nurses (RMN and RNLD), activity coordinators, support workers and is led by a psychiatrist who acts as our responsible clinician.
- The people we support benefit from Guided Mindful Meditation (GMM) group sessions: a concept used with huge success in teaching focus and control; regulation of emotions; mobilisation of inner resources; and retraining the brain.
- Individual access is provided to GP services, pharmacology, optician, chiropody and annual health screening.
- Stringent risk assessment and management is key, allowing for daily community access and inclusion: sport, education, the arts, life skills, volunteering and work experience.
- Moving-on programmes: we provide a unique opportunity to move through individual personalised accommodation to pre supported living flats before moving into the open community, where we can also provide Outreach support.
