Complex Care - Learning Disabilities/Autism
AtPersonalized Care Services we believe people with a learning disability and/or autism have the same right to opportunities as we all have to live a satisfying and valued life, and to be treated with dignity and respect. People should have a home within their own community, be able to develop and maintain relationships, and get the support they need to live a healthy, safe and rewarding life.
Personalized Care Services staff with their wealth of experience and dedication have advocated for and supported people with a learning disabilities and/or autism, to lead happier lives. We know from the experiences of people using our services that this is achievable. Personalized Care Services adopts a holistic approach working with people with a learning disability and/or autism, their families, staff, clinicians, other service providers, and commissioners.
We support people to:
· Maximise their independence and wellbeing in a person centred manner
· Understand and apply social care values at all times
· Enable and encourage people to access universal services and community facilities in their own area
· Manage an acceptable level of risk in their day-to-day lives
· Support people in managing their personal affairs and finances
· Identify and fit simple equipment to maximise people’s independence
· Develop communication, social interaction and imagination
· Improve on self-esteem, pride in accomplishments
· Improve recognition of emotions for the individual and others
· New leisure time activity in a group where everyone can be successful
· New awareness of volume levels
· New skills for functioning as part of a group
· New skills for following direction
· Understanding emotions through body language
· Giving space to express as individuals whilst gradually reinforcing realistic personal targets you set with them
· Working with the local community
· Learning appropriate social rules and keeping safe
We will aim to create a safe environment, supporting and enabling each person to gain the skills and confidence to become part of their community in which people with complex health and social care needs live and feel both valued and respected. We will create valued social roles for people with complex health and social care needs. We continue to develop opportunities for people to become active members of their local community, to enable them to develop new skills, enjoy new experiences, and thus build greater confidence and self-esteem. Support to develop opportunities for people to spend their leisure/recreational time in mainstream community venues. We will strive to offer a choice of opportunities for people who wish to spend their leisure/recreational time experiencing a number of different activities. Daytime opportunities will make full use of mainstream facilities, supported employment, volunteer work, adult learning and other meaningful day time occupations within the wider community where people with complex health and social care needs can feel valued as respected participants.
How we will deliver person centred support
· Providing person-centred care is about the individual’s goals and aspirations for their life, what are their positive qualities, how they wish to be supported, who and what is important in their life
· We envisage support that will engage people to have more control over their lives, and experience greater inclusion
· We believe in focusing on people’s strengths. This sometimes involves pushing boundaries, changing and challenging perceptions. We will encourage positive risks to develop and grow individuals, whilst always safeguarding people from harm
· We recognise some individuals will need specialist support, we plan to work in partnership with other professionals, to develop our staff through high quality training, to ensure they have the skills and experience they need to help people with multiple needs
