This course covers all you will need to know to have a basic understanding of personalisation and the process of self directed support.

Aims
To provide all the information you would need to have an understanding of personalisation and the process of self directed support. We aim for all participants to leave the day feeling confident within the role they have to implement personalization and self directed support, enabling you to move forward in achieving successes for yourself and people you know.

The course will cover both technical aspects of self directed support as well as introductions to some people who have achieved this for themselves and what they experienced in the process. We aim to jargon bust and deliver a learning experience that people can understand easily. As trainers we encourage everyone’s involvement in the learning experience, and to fully participate in exercises to gain the most from the day. We believe that people can learn as much from each other on the day and therefore deliver the training with discussion as a large portion of the content….. This will not be a Powerpoint overload day.

• How to make a referral for a social care assessment
• An understanding of the process from referral to outcome
• How to develop a meaningful support plan
• What needs to be included in a support plan
• An ability to highlight the important stages of the process to ensure a person receives what they are entitled to
• You will have a better understanding of the terminology and how this is being used differently within different local authorities
• An insight into real life experience of Personalisation and Self Directed Support

Who is this for
This course is for anyone with an interest in understanding the personalization process and how to achieve self directed support. This could include individuals, families, community groups, people work within service provision, private sector or statutory. It would also be of interest to anyone working alongside people who access social care support, people with learning disabilities, mental health issues, in dementia care, physical disabilities.
