Mental Wellbeing
AtPersonalized Care Services we aim to make a positive difference to the lives of people who suffer from mental health conditions. We use a holistic approach working in partnership with people and their families and other health care professionals to build meaningful links and develop positive strategies that facilitates successful reintegration into the local community. Our support is based on an integrated approach having all the people important to the individual with health and social care professionals are working together to support the person lead a happy life. Personalized Care Services believes people should be able to get the appropriate support to live fulfilled lives – with reasonable adjustments made to their support aimed at reducing or preventing anti-social or ‘offending’ behaviour, and a community forensic health and care function to support people who may pose a risk to themselves and others in the community.
Individualized tailored support for people who:
· Have a mental health condition such as severe anxiety, depression, or a psychotic illness, and those people with personality disorders, which may result in them displaying behaviour that challenges.
· Display self-injurious or aggressive behaviour (not related to severe mental ill health), some of who will have a specific neuro-developmental syndrome where there may be an increased likelihood of developing behaviour that challenges.
· Display risky behaviours which may put themselves or others at risk and which could lead to contact with the criminal justice system (this could include things like fire-setting, abusive/aggressive or sexually inappropriate behaviour).
· Often have lower level support needs and who may not traditionally be known to health and social care services, from disadvantaged backgrounds (e.g. social disadvantage, substance abuse, troubled family backgrounds) who display behaviour that challenges, including behaviour that may lead to contact with the criminal justice system.
Our person-centred approach encourages empowerment and well-being, it enables people we support to take personal responsibility with the primary focus on community integration. The recovery is based around social inclusion, and an increased involvement in meaningful local community-based activities/events.
