A full 5 day training course covering the history and the principles of person centred planning. During this course you will gain practical experience of the following person centred planning tools MAP, PATH and essential life style planning. We will also discover how person centred planning techniques can be utilised within self directed support.

Learning Outcomes
• A history on the principles of Person Centred Panning
• Lets go back to the beginning where it all came from and learn how person centered planning can be used in a meaningful way
• How to create a plan that is right for the person
• How to create a plan that can be implemented with the person’s Circle of Support
• How to develop and set up a Circle of Support
• An understanding of the right and wrong ways to do this
• The many ways a person centered plan can be broken down into understandable pieces so the person can add freely to their plan
• Ensuring that the plan remains a living plan
• Learning techniques for you to get the best outcome
• Letting the persons dreams and aspirations be the lead on this and how to make this happen

This course is for anyone who is wanting a practical understanding of person centered planning and to be confident in the use of Person Centred Planning techniques. This could include individuals, families, circles of support, community groups, people work within service provision, private sector or statutory. It would also be of interest to anyone working alongside people who access social care support, people with learning disabilities, mental health issues, in dementia care, physical disabilities.
