This course will introduce the principles of person centered planning with an overview of the approaches, the connection between person centered planning and community / inclusion and the development of a support plan. We will also introduce some practical techniques for you to take away and use.

Learning Outcomes
During the day we will cover:
• An overview of effective approaches to achieve personalisation
• The importance of community connecting
• Practical Person Centered Planning techniques, i.e. Communication Map, Good Day / Bad Day, Community Map
This will all be achieved by you actively taking part in a 1 day programme that will help you to understand the importance of Person Centered Planning and the awareness of making this into a real life plan that can be carried on through the persons life.

This course is for anyone interested in the techniques and practices used throughout Person Centered Planning and the outcomes that can be achieved using the right tools to over come the barriers in someone’s life. This could be for yourself, a family member or friend. It would also be of interest to anyone working alongside people who access social care support, people with learning disabilities, mental health issues, in dementia care, physical disabilities
