Description
During this course we will look at the criteria for a support plan within self directed support, we will demonstrate how it is possible to meet the criteria whilst maintaining a person centred approach. We will explore the person centred planning techniques that, when used, can be a big step in developing a meaningful and significant support plan.
Aims
Creating a Support Plan is the pivotal point of getting it right. The aim of personalisation and self directed support is wonderful and a breath of fresh air for many people, although we still have to remain thoughtful at each step of the way to ensure that the person is taking the lead and that the outcome achieved is what they would want. Ultimately to avoid an approach that is overly systemized without a focus on the individual.
Each person has their own uniqueness and we aim to show you during this one day course how to capture this uniqueness in the documents that need to be completed to achieve self directed support. Developing techniques to listen closely and develop a collective approach to getting it right.
Learning Outcomes
• You will develop and appreciation of how to remain focused on the individual
• You will develop a perspective that encourages you to create a flexible system / process that works for the individual
• How to ensure that the support plan contains all the information that is required by the local authority to agree the use of the budget whilst ensuring the person and their circle of support remain in control
• You will develop an understanding of what stage of self directed process the Support Plan is needed.

This course will be of interest to anyone who is writing their own support plan or if you are or will be supporting another person to write a support plan. This course could be for yourself, a family member or friend. It would also be of interest to anyone working alongside people who access social care support, people with learning disabilities, mental health issues, in dementia care, physical disabilities.
