Outreach Service
(One to one or small groups, if desired)-to be available on an hourly cost basis so the service users can be assisted to achieve specific goals as identified in the care plan.

You may wish to use this service to access community activities, or to aid rehabilitation, gain life skills, e.g. learn to shop on line, join a club or learn a new skill in order to increase your independence and confidence.(Operating up to seven days per week subject to demand - excluding Bank Holidays)

This service is available up to 7 days a week. The hourly rate ranges from £12.00 - £15.00 per hour, the exact rate will be confirmed to you once we have discussed and established your needs and requirements.
